Consent Agenda - CONSENT AGENDA:
1. Approve the following nominations: Mr. Darryl T. Bey, Chairman; Ms. Glynda W. Cavalcanti, Vice Chairman; Mr. Mr. Daniel M. DeIulio, Secretary; Mr. Charlie Frank Matthews, Deputy Secretary; and Mrs. Linda Hudson, Mayor/Member.
2. Bid No. 18-61: Approve award of Bid for the purchase of 60 enclosures and 80 handholes from Electric Supply of Tampa, Inc. dba Electric Supply, Inc. of Tampa, Florida, in an amount not to exceed $43,692 annually. The initial term of the PO Contract will be from October 3, 2018 to October 2, 2019 with four one-year renewal options.
3. POA 16-40: Approve the Contract with Membrane Treatment Services LLC, of Fort
Pierce, Florida, in an amount not to exceed $105,000, for the single source purchase
of PLC and SCADA Network Programming and Maintenance Services. The term of this
contract will be from October 3, 2018 to October 2, 2021.
4. Approve Post Budgeted Project/Budget Request in the amount of $40,000 to replace
FPUA's gas main in order to relocate it, for the City's Mayflower Canal Phase III project.
5. POA 14-53: Approve the sole source purchase of specialty sewer line camera
equipment and repairs for existing TV equipment from ELXSI Corporation dba Cues,
of Orlando, Florida, in an amount not to exceed $30,000 annually.

Recommendations prepared by Relation Insurance Services of Florida, Inc., of Stuart,
Florida. b) Accept the Stop Loss Proposal from Blue Cross and Blue Shield of Florida,
Inc. dba Florida Blue, of Jacksonville, Florida, and administered by HM Life Insurance
Company, of Pittsburgh, Pennsylvania, for stop loss insurance coverage effective
January 1, 2019 through December 31, 2019, and approval of premium funding of
$717,617. c) Approve payment of administrative fees to Blue Cross and Blue Shield of
Florida, Inc. dba Florida Blue, of Jacksonville, Florida, in the amount of $63.28 per
employee per month estimated to be $207,305 based upon enrollment of 273
members, subject to enrollment fluctuations, for the period from January 1, 2019
through December 31, 2019. d) Approve the BlueMedicare Group Master Agreement
with Blue Cross and Blue Shield of Florida, Inc. dba Florida Blue, of Jacksonville,
Florida, for the provision of Medicare Supplement plans to the retirees of FPUA. The
premiums for these plans are fully paid by the retiree, and are not expected to exceed
$45,000 annually. The term of this agreement will be from January 1, 2019 through
December 31, 2019, with unlimited automatic one-year renewal options.
