Consent Agenda - 1. POA 18-27: Approve the sole source purchase of Lime Slaking Equipment in an amount not to exceed $149,900 from Integrity Municipal Systems LLC of Poway, California.
2. Approve of no change to the Purchased Gas Adjustment (PGA), September 2017.
3. Approve appointment of Craig Crawford to the position of Gas Operations Superintendent, Gina Morris to the position of Customer Service Manager, Rachel Tennant to the position of Manatee Observation and Education Center (MOEC) Manager and Jann Widmayer to the position of Community Relations Manager.
4. Approve the payment to the City of Sturgis, Michigan in the amount of $72,431.53 for Mutual Aid assistance after Hurricane Irma.
5. Approve the payment to Marquette Board of Power and Light of Marquette, Michigan in the amount of $85,544.44 for Mutual Aid assistance after Hurricane Irma.
7. POA 18-13: Approve Agreement for Administrative Services Only Dental Administration Fee Structure with Florida Combined Life Insurance Company, Inc. for a not-to-exceed amount of $21,998 annually, based on a two-year renewal rate of $6.05 per employee per month, from January 1, 2018 to December 31, 2019.
8. POA 18-17: Approve the Amendment to Administrative Services Agreement with Blue Cross and Blue Shield of Florida, Inc. dba Florida Blue, of Jacksonville, Florida for medical health plan administrative services for a not-to-exceed total of $206,546 annually. The term of this Amendment will be from January 1, 2018 to December 31, 2021.
9. RFQ 18-24: Accept quote from and approve Contract for construction of the Island Water Reclamation Facility (IWRF) Canopy Building with Richard K. Davis Construction Corp, of the City of Fort Pierce, Florida, in an amount not to exceed $58,916, for the period of November 22, 2017 through March 31, 2018, contingent upon receipt of the required Certificate(s) of Insurance.
